PO-05-035 FASCICULAR SUBSTRATE MODIFICATION TO TREAT HUMAN VENTRICULAR FIBRILLATION: A MULTICENTER STUDY

نویسندگان

چکیده

Purkinje fibers play an important role in initiation and maintenance of ventricular fibrillation (VF). Fascicular substrate modification approaches have been suggested to treat recurrent VF isolated case reports small series. To investigate the outcomes catheter-based fascicular VF. Out 2,212 consecutive patients with arrhythmia undergoing catheter ablation at our centers, 18 (0.81%) underwent by targeting short (<20 s) RF lesions most distal segment as identified high-density mapping during sinus rhythm. Demographic, clinical, follow-up characteristics were prospectively collected institutional database. A total (mean age: 56±3.8 years, female: 4, 22%) included study. Of those, 11 (61.1%) had idiopathic (normal heart VF), 3 (16.7%) non-ischemic cardiomyopathy 4 (22.2%) mixed cardiomyopathy. Average LVEF was 42.5%. Patients failed ≥2 antiarrhythmic drugs pre-ablation. At baseline, all inducible non-inducibility end procedure achieved 16 (88.9%) patients. In select patients, remapping revealed elimination targeted potentials. procedure, no patient demonstrated new evidence block or bundle branch block. final lesion sets are shown figure 1. Modification of Ventricular Fibrillation

Abrupt occlusion of the left anterior descending coronary artery was performed on 43x unanesthetized farm pigs in order to evaluate the relative effects on the latency to ventricular fibrillation (\VFL) of 1 ) adaptation of the animals to the laboratory, and 2) beta-receptor blockade by propranolol. Compared to control values, VFL, was greatly lengthened (i.e., VF delayed or prevented) by adapt...

Trigger elimination of polymorphic ventricular tachycardia and ventricular fibrillation by catheter ablation: trigger and substrate modification

Ventricular fibrillation (VF) is a malignant arrhythmia, usually initiated by a ventricular premature contraction (VPC) during the vulnerable period of cardiac repolarization. Ablation therapy for VF has been described and increasingly reported. Targets for VF triggers are VPCs preceded by Purkinje potentials or from the right ventricular outflow tract (RVOT) in structurally normal hearts, and ...

Idiopathic Fascicular Ventricular Tachycardia

Idiopathic fascicular ventricular tachycardia is an important cardiac arrhythmia with specific electrocardiographic features and therapeutic options. It is characterized by relatively narrow QRS complex and right bundle branch block pattern. The QRS axis depends on which fascicle is involved in the re-entry. Left axis deviation is noted with left posterior fascicular tachycardia and right axis ...

Fascicular Ventricular Arrhythmias

Ventricular arrhythmias involving the fascicular system may be seen in both structurally normal and abnormal hearts. Idiopathic fascicular ventricular tachycardia represents almost 10% to 15% of idiopathic ventricular tachycardia related to the left ventricle. Cohen et al and subsequently Zipes et al first described these arrhythmias in the 1970s as relatively narrow right bundle left axis arrh...

Human Ventricular Fibrillation

Amajor controversy in cardiac electrophysiology is whether ventricular fibrillation (VF) is maintained by wandering wavelets or mother rotors. On the basis of a computer model of atrial fibrillation by Moe1 developed in 1962, fibrillation was hypothesized to be maintained by multiple small, wandering wavelets, in which the activation pattern is constantly changing. According to the wandering wa...
